Not being able to get a 3 and out is not "failing". It's putting your defense in a ****** spot. I'd wager the NFL's 3 and out percentage is lower than the percentage of getting 10 yards on 4th and 10.

It is pretty funny watching the same people blast Marrone for being conservative (on a call that worked by any reasonable measure - the Bills had the ball back 35 more yards upfield 2 minutes later), then blasting him for being aggressive at the end.
