My guess is that it was an alteration made out of an excess of caution. Referring to any Black man as a "boy" in any context can be taken very negatively. I don't think Sam would have meant anything by "birthday boy," but I also don't doubt that MSG might have guidelines on such things just to avoid any opportunity for offense.
