Breakaways tend to indicate that your team is making poor decisions with the puck. That's the fault of the coach. Leaving a guy standing right in front of the goalie with no one within miles to defend him? That's the fault of the coach. When players don't know where the f*** on the ice they're supposed to be, night in and night out, and it's losing you games, that's the fault of the coach. If the players are where they're supposed to be and they're still losing games on a consistent basis, that means the system is faulty- and that's the fault of the coach.
