I suspect there is a whole myriad of factors, including:
  • population density
  • travel patterns
  • demographic stratification (age, presence of comorbidities, percentage living in nursing homes, etc.)
  • healthcare system organization and capacity
  • degree and timing of government response (lockdown, tracking, isolation, quarantine, border closings, etc.)
  • adherence of the population to government response
  • geographic isolation
  • pure dumb luck

It seems like it spread pretty strategically in the beginning. China, Iran, and Italy were the first countries with an out break. Look on a map they are almost perfect spaced out.
