When I remember me right from the EHM Editor there is a first and second Nationality and I think it takes the first one, but 100% I am not sure and I suggest better to set only one Nationality.
Maybe has Francis a clear answer.

So, in Football Manager, a player with dual citizenship can play for the team that he gets called up for. His nationality then gets changed over.

In EHM, I noticed that even when the player gets called up for his secondary nation, the flag doesn't change. I have Portugal playing in the world championships in my game, and I have many Finnish and Swedish players that I edited to have the Portuguese nationality with. Even after they play official games, their flag never changes over to be 'officially' Portuguese.

They usually join which ever team calls them up first

One thing to be mindful of though is sometimes in choosing those countries you never get an opportunity to get selected
