How common is it for a player to play in all three leagues during one contract, any examples?

I think you are it with goalies mostly. Joni Ortio a few years ago for example, Jonathan Quick going back like 10 years ago now. I'm sure there's more examples it those two are off the top of my head

Players on non-elc NHL deals cannot be assigned to the ECHL without their permission anyways, so I can't see a situation where this would ever happen anymore.

If someone is bad enough that their team wants to stick them in te AHL, they'd be better off with a contract termination and going overseas.

From the CBA:

9.6 Loans to East Coast Hockey League. Players who are party to an Entry Level SPC may be Loaned to the East Coast Hockey League ("ECHL") without the requirement of Player consent, provided the Player will continue to be paid the stated amount of the AHL portion of his two-way SPC.

So no, there won't be any "ECHL salary" for players with NHL contracts until at least after this CBA expires in '22.

Brett Kulak in his first Pro season in 14/15. He played 39 games in the ECHL and put up 30 points so they called him up to the AHL and he was decent enough there to get into 1 NHL game at the end of the season.

Such used to be allowed, but I don't believe it's been allowed since the CBA after the 2004-05 lockout. The current CBA only stipulates AHL pay (although a player on an ELC can be assigned to the ECHL, but on AHL pay).
