In Richards case he wasn't in the player assistance program nor did the league suspend him.

If Nuke was given a lifetime ban, I doubt the Avs would suffer any sort of cap penalty.

Brantt Myhres was given. lifetime ban for failing multiple drug tests, AFAIK that was first and so far only time such a thing happened.

that was different. LAK did not wait for the court to decide things and that is why they terminated him and later got hit hit with the penalty.
