In 1978 the 49ers fumbled the ball 10 times and lost 4 of them.
In 1967 the Lions fumbled the ball 10 times and lost 5 of them.
Most lost appears to be the Browns in 1990 who fumbled 9 times and lost 8 of them.

Most by a player in a game:
Chad Pennington 2005 - 6 fumbles 2 lost
Kurt Warner 2003 - 6 fumbles 3 lost
Most lost appears to be Trevor Lawrence last year who fumbled 4 times and lost them all against the Eagles.
