I'm not sure I'll ever stand NHL rules on coincident minor penalties. Maybe I'm just remembering wrong, but sometimes they result in 4 on 4 and sometimes they don't, with no discernable pattern. USA Hockey (which I officiate for) never reduces on-ice strength for coincidental minors (or majors) though.
