We could be looking the red tier in a few weeks if the Case rate declines fast enough
Something we caught the other day was that Infectious Disease Doctors, who follow on these Covid Cases, prescribe based on Pt response etc,. So after 2 to 3 weeks, whatever the hospital standard is and the decision making there within, the Pts are still hospitalized but FALL OFF the COVID numbers because they are not actively infectious so it decreases the number of COVID cases but they are still there for COVID related diagnosis like ARDS. I feel like the numbers dropping is a little misleading because it doesn't tell the full story of how the hospitals are doing. I have had a number of patients who now have tracheostomies, which is a breathing tube in the throat secondary to being unable to wean off the vent. We don't even retest them to see if they are positive. This was pretty surprising to me and unnerving so I'm still wearing an elastomir on those patients if they are not aerosolizing but if they are spewing viral particles because of an airleak in the tube, then back to the PAPR with an N95 underneath.

I mentioned before that I felt February was going to be the month the numbers would drop but I think by month end we are going to be further along that path unless one of the variants out there really amps up and vaccinations get stalled. Already my unit was 75-85% COVID and now it's down to about 60%. That means current, active infection. The others are still there for COVID reasons but because they are not COVID infectious, they don't fall within those numbers. So basically, yes the positivity rate is going down, but I don't know how to quantify that because they are still the same COVID patients who require PPE and the unit is still full.

So basically, our acuity is high because these patients are very sick, but they are going out of the infectious phase into the fibrotic phase. I just wanted to put an asterisk in there. The number of new infections seem to be going down which is great and no major holidays for awhile so should be interesting in the Spring with the new variants out there.

Seems like my SPED department is gearing up to return to campus once again. We were back for a month in Oct.-Nov. before they sent us online again. Good thing because multiple students and employees have got it since then. Hope the cycle of open up, surge, and close again, can be mitigated by the vaccines.

I had to run up to the grocery store today to pick up a few things (milk, eggs, bread), and I saw a number of couples with full carts, and the butcher counter was crowded. It was almost like something big was happening on Sunday.

I think the real problem is unlike MacArthur advocating the invasion of China, General Covid is Chinese and invaded the rest of the world.
