Just go to MS Word and make a table or something with 3 columns and 4 rows
Then make another table of 2 columns and 3 rows. Hope that's what you mean.

8x10 printable, right click and save image AFTER opening it full size
Can edit it to your team colors and add logo if you would like. maybe change header
for head coach to Captain? Can also put it into Italian if you provide the translations.

Here is the version updated with the requests made. IF there are any others feel free. I saved the original in .psd so the next edit will be much easier.

when printing you might want to SET TO PRINTER MARGINS in your print options in order to get the same look as it shows in Adobe PDF reader. If you want a matching Roster and/or stats sheet, give me the details and I will put it together into a several page PDF file

page 1 Full size with white BG and page 2 of the PDF is 4x5. Not a problem...
anyone else need forms let me know. I have goalie evaluation forms and player evaluation for try outs, stats, Blank Drill Diagrams etc etc. Can customize them to your team colors w/logos etc etc.
